Gosforth and Jesmond Health has an opening Care Co-ordinator to join our multi-disciplinary team providing NHS care to patients at GP practices in Gosforth and Jesmond.
About the Role
The role focuses on coordinating and developing two new services delivered to patients at Primary Care Network level. These are the Enhanced Access service, which provides additional evening and weekend appointments, and the Same Day Access service where our Paramedics offer appointments for people with acute needs.
The Care Co-ordinator will act as a central point of contact to ensure these services run smoothly and patients receive the best possible care.
This will be a varied but pivotal role in developing these services as part of our wider vision for integrated community healthcare services for patients at GP practices.

About You
We are looking for a motivated individual with the ability to organise and prioritise their workload and attention to detail needed for regular reporting, peer reviews and other evaluations.
Essential skills and experience include organisational skills, written and verbal communications, knowledge of local health and social care provision and a relevant vocational qualification at level 4 or above, eg NVQ L4, Cert HE, HNC, L4 Award/Cert./Dip.
Experience of working in a similar role, using clinical systems such as SystmOne and EMIS web, and a formal safeguarding qualification would be desirable.
