We have a vacancy for an Integrates Care Clinical Pharmacist or Lead Clinical Pharmacist to join our Primary Care Networks teams in Jesmond and Gosforth.
About the Role
The successful candidate will be joining an established team of Pharmacists and Pharmacy Technicians in a patient-facing role. You can make a real difference for multi-comorbid individuals and those at risk of medication-related harm through a person-centred approach.
You will be supported in practice and on the Primary Care Pharmacy Education Pathway by a named GP, a senior clinical pharmacist and an educational supervisor from CPPE as well as our talented, research-active multidisciplinary teams.
Job responsibilities will include:
- structured medication review in line with the NHS England Network DES
- multidisciplinary meetings and reviews
- integration with local community pharmacies and the acute Trust to improve patient care
The post will be resources and supported by the Newcastle upon Tyne Hospitals NHS Foundation Trust but will be based across the four GP Practices of the Jesmond Lower Gosforth Primary Care Network (JLG PCN) and the four GP Practices in North Gosforth Primary Care Network (NG PCN).

What we are looking for
You will have an M Pharm (or equivalent degree course with pre-registration experience and be registered with the GPhC. In addition to excellent communication skills and the ability to work effectively as part of a team, you will have significant post registration experience in Hospital Pharmacy, Community Pharmacy or Primary Care Pharmacy.
This is an evolving role and we are looking for an innovative, motivated person to join the team.

About JLG and NG PCNs
The two networks work collaboratively to maximise efficiency. Each PCN has four member GP practices.
This collaborative approach means that specialists from each network can work together, sharing best practices and raising standards, and teams can collectively provide the best holistic care for patients.
The networks also share a Strategic Manager who oversees the day-to-day running of both PCNs, as well as other supporting functions across business intelligence, IT, communications and project oversight.
