Jesmond Health Partnership is looking to recruit an individual to work as part of our Acute Care Team. The individual may be an Advanced Nurse Practitioner, Emergency Care Practitioner or Paramedic Practitioner, preferably with experience of working within Primary Care.

About the Role

The role is part of a wider skill mix we are developing within our team. We are looking to innovate and develop a new model that benefits our patients and staff.

The successful candidate will play an important part in expanding our urgent access on a daily basis, including in-practice consultations and home visits. This is part of a proactive approach to managing the health of our housebound/frail population.

What we are looking for

You will be an independent prescriber (non-medical), with experience in primary or community care. A good communicator, you will be caring, sensible and hardworking, and confident to work autonomously and manage your own clinics and workload within a supportive team.

About JHP

We operate two practice sites less than a mile apart in Jesmond, a suburb of Newcastle upon Tyne, where we have a combined list of around 13,500 patients.
