Asking to See Your Health Records (Subject Access Request)

You have the right to see your health records. This is called a Subject Access Request (SAR). You can ask for a copy of your records from your GP. These may include notes from appointments, test results, and letters from other services.

Please note: A Subject Access Request is not the same as requesting online access or the NHS App.

It is usually free to make a Subject Access Request. We will reply within one month, where possible.

You do not have to give a reason, but it helps if you tell us what you need – for example, a date or the name of a doctor.

Guide to Submitting Subject Access Requests

Before submitting an SAR, please read our guide below.

  • Anyone aged 16+ can request to see their medical records.
  • Parents or guardians can request records for children under 13. (Ages 13-15 usually need to give consent).
  • Third Parties, like insurance companies or Solicitors, can request records with your approval
  • It is free
  • Being specific with your requests helps us deal with your request efficiently.
  • You can let us know a date range and specific events you want to include.
  • Your GP record can included consultation notes, diagnoses, treatment plans, test results, referral letters and medication history.
  • It may include information about your health that you find upsetting
  • It may not include details about other people, or information that we are not data controllers for (ie hospital test results or safeguarding information.
  • We may also exclude information we have a legal duty to protect, or if we believe it could cause serious harm to you or someone else.
  • You can ask for paper or digital copies
  • We have 1 month to respond (but we can extend to 3 months if necessary)
  • Your first Subject Access request is usually free. Further requests may be chargeable.
