About the Role
Social Prescribing is about supporting and empowering people to be able to take a proactive role in managing their own health.
The role holder will work largely with our older population, particularly those at home, and connect them to the many services in the NHS and wider community that can support them and their general wellbeing.
The role is about understanding what matters most to the individual and working to create a personalised plan with them that helps achieve their goals.

About You
We are looking for someone with previous experience working in Social Prescribing, or a similar role, preferably with people aged 65 and above.
You will have the essential skills to manage and prioritise a caseload, and work flexibility and enthusiastically within a team and on your own initiative.
Knowledge of primary care, local health and social care provision, available funding streams and an understanding of the needs of vulnerable adults and safeguarding are also essential.
As well as excellent people skills, and the ability to work and communicate effectively with people in a range of situations, experience with minor clinical observations (blood pressure, temperature, oxygen saturation) and clinical systems such as SystmOne or EMIS web would be advantageous.
