Jesmond Health Partnership is looking for an enthusiastic, proactive individual to join our team as a Social Prescribing Link Worker.

About the Role

The role is part of evolving our team to meet patients’ needs. Social Prescribers help people with non-medical issues, connecting them to services and helping to empower people.

We are recruiting for a Social Prescribing Link Worker who will specialise in identifying patients who may not yet be able to take full advantage of the many and excellent digital services offered in the NHS and the wider community.

The successful candidate will play an important, proactive part in helping people build the skills and confidence to access services, events and organisations that will genuinely benefit their health and wellbeing.

What we are looking for

This role would appeal to a creative problem solver, someone who is able to listen and ask questions to gain a full understanding of a situation, formulate a plan to tackle the issues, and help individuals overcome barriers. The ability to support people through using new digital technologies with guidance and encouragement would be a strong asset.
