New patients are always welcome at Jesmond Health Partnership.
Before you can book an appointment, you will need to register with us. Here’s the 4 step process to become our newest patient:
1 – Check you live within our boundaries by entering your postcode below:
This form combines a ‘GMS1’ form, an NHS document required to trace your medical history, and our registration form to help us get to know you better.
If you are using a mobile device, you may need to install an ‘Adobe Acrobat Reader’ or ‘Fill and Sign’ app to complete the form.
Make sure the form is downloaded to your device – it currently won’t submit through web browsers.
3 – Once you’ve filled in your details, click ‘Submit’ on the form.
This will attach your completed form to an email addressed to us. All you need to do is click send.
If you encounter a problem with this method, please save your completed form, and send it by email to firstname.lastname@example.org
4 – The final step is to provide photo identification
Normally, we’d ask you to visit the practice whenever you can, but as we’re implementing social distancing to reduce the risk of infection, we will arrange to do this at a later date. Once we have processed your registration, you will be able to use our services.
Beyond the current health pandemic, we intend to work with NHS digital to look at ways of streamlining the patient registration process, in particular making it all available online.
