Our model
The PPG is a partnership between the JHP and its registered patients. Members are all volunteers and give their time for free.
We support the practice to maintain and improve good standards of care for us all.
We are keen to make sure everyone is represented and can engage and participate in our work.

Membership of JHP PPG
At JHP, there are two ways for patients to get involved – as Friends of the PPG and Full Membership (see below).
Joining as a Friend or a Member is open to patients over 18 registered with the JHP, carers of patients registered with the practice, even if registered elsewhere themselves, and practice staff.
Friends of JHP PPG
As a Friend, we’ll keep you in touch with what the PPG is doing, what’s happening and changes at the Practice, our local NHS and national issues. These will be shared in our quarterly newsletter and on the PPG pages on the practice website.
Your opinions are important so we may invite you to take part in patient surveys or consultations.
You could also take part as a volunteer in PPG projects.
You can retain your Friends membership as long as you are registered with JHP, or until you choose to resign.
To join as a Friend of JHP PPG, please fill in an application form online hot link, or on paper (available from reception desks) and leave it at the reception desk marked PPG.
Full Members of JHP PPG
If you would like to be more actively involved in the work the PPG does, you can apply for Full Membership of the PPG at any time.
You can remain a Member as long as you are registered with JHP, or until you choose to resign. You can step back from full Membership at any time, but still stay in touch as a Friend.
As a full Member you will be able to
- keep up to date with what’s happening at the Practice (all four surgeries), wider local NHS services and national changes that have to be implemented
- ‘Have Your Say’ in how services and processes in the practice are running, and help identify any problems or changes needed
- take part in PPG activities such as
- PPG patient surveys and consultations
- contributing to feedback about the patient experiences and making suggestions for change
- promoting and supporting practice activities e.g. the annual flu vaccination campaign
- attending information sessions to understand better how services work and how to use them effectively
- contributing to the PPG newsletter
Members may be able to develop new skills and understanding which could be useful in particular to students in health and social care.
To join as a Member of JHP PPG, please fill in an application form online (hot link), or a paper copy (available from reception desks) and leave it at the reception desk marked PPG in one of the surgeries.
The personal details disclosed on applications for both Friend and Membership are stored securely as per JHP confidentiality guidance. Applicants are asked to indicate how the PPG can keep in touch with them and this will also be managed securely and individual contact details will not be shared.
All applicants to join JHP PPG will be contacted by the Steering Group Chair to confirm their acceptance onto the group
Managing the work of the PPG
The Steering Group
The work of the PPG is managed by a small Steering Group drawn from the PPG full Members. As volunteers they give their time for free.
Invitations to join the Steering Group are issued when a vacancy arises. Appointment to the Steering Group is for three years, but Members can reapply at the end of that term for up to three terms.
The Steering Group meets bi-monthly with senior members of the Practice on behalf of the wider PPG membership to discuss, for example,
- an update on the practice – new initiatives, changes or requirements
- implications for changes to services
- issues arising from patient feedback
- projects or support the PPG can offer the practice
Any Member can suggest agenda items for the Steering Group meetings.
The agenda and notes of these meetings are available on the PPG pages on the practice website. Meeting outcomes are also shared via the PPG quarterly newsletter.
Individual members of the Steering Group may lead activities such as organising an event, co-ordinating a survey, or leading PPG support for a practice event, but this is not a requirement.
PPG Chair
The Members of Steering Group elect a Chair who also serves for a 3 year period.
The Chair manages the business of the PPG, organising meetings, managing discussions at the meetings, and making sure the meetings are recorded and shared.
The Chair is the main point of contact for the PPG.
